Join CHPV, UChicago Medicine, and the SHARE Network for an information session on Advanced Care Planning! Advanced care planning is the process of preparing for future decisions about your medical care if you become seriously ill or unable to communicate your wishes. Participants will have the opportunity to sign up for one-on-one consultations with a SHARE Network team member in January. Complete advance care planning documentation and share them with family members and healthcare providers.
