Join CHPV and the SHARE Network for a hands-on workshop as UChicago medical residents help you navigate how to "preparing for your doctor visit." Review the 4Ms (medication, mentation, mindfulness, & mobility) and discuss key talking-points, including medication, provider info, contact information, and even personal goals.
At this community health event, you'll have the opportunity to:
• Reflect on What Matters most to you, which will help you to better prepare for your doctor's visit
• Understand how to review your Medications before your visit
• Learn the importance of Mobility and create goals to stay active in ways you enjoy
• Prepare activities that engage the Mind to help you reduce stress and anxiety
• Work with UChicago medical students
