Prepaid reservation is required. Only those with a prepaid reservation may attend. We are unable to accommodate walk-ins.

- Seating is limited; if the seating limit is reached call the CHPV Office (773-363-1933) to be placed on a wait list and be contacted if a space opens up to register, select entrée, & make payment
- We will be seated at two tables of 10 each
- Reservations & payment close Monday, March 13th at 3:00 p.m.
- On-Line reservation paid with a credit card (preferred method - a Convenience Fee Charge of $2.00 automatically added to the credit card payment).
- Reservation, choice of entrée, and payment by cash or check can also be made at the CHPV Office (773-363-1933)
- Please cancel your reservation by contacting Terrill Stumpf at tls1649@sbcglobal.net if you are unable to attend - no refunds for cancellations after Tuesday, March 13th at 3:00 p.m.
